DESCRIPTION Additional miscellaneous repairs requested by customer while the vehicle was in the shop for damage repair from accident VIN 4UZACMFDONCNR9688 UNIT LOT (S) TERMS Upon Receipt TOTAL USD $3,500.00 --storage closet latch needed tightened. -the bathroom door handle and the hallway door handle hit; not much I can do with this; it's how the truck was designed --latch on screen door is not letting the entry door open without getting caught on the frame; we adjusted catch --left side cabinet in exam room won't stay closed; adjusted the latch on door so it catches -kick panel on hall door between front room and middle room is coming off; we refastened it back on -seal around driver door is loose; pulled seal and replaced it. seal loose at wheelchair door; replaced it --screen door panel off; installed it --the compartment door that houses the removeable steps needs a new strut; replaced strut -graphics are peeling around driver's door in stepwell; not a lot we can do about this. when the door's closing it rubs the step well causing the graphic to peel; cut off the excess loose graphic and installed new grip tape but made it where it covers the graphics --ceiling speakers inop. the amp is in protection mode; amp out bench tested will not come out of protection mode. needs replaced. we replaced the amp as we were in test mode when we get to the middle room volume control as soon as we touch it the audio system cuts out. pulled volume control found that while the truck was in production they had to cut out the wall so the volume control is shorting out in the wall where the structure was cut into causing the amp to go into protection mode. we had to cut back the tube in the wall to clear the volume control and then reinstalled it and tested; now everything is working. --replaced the weather stripping on top of the driver door --pocket door in rear room bottom is busted out; we had to pull the wall apart on both side to get the pocket door out; had the door remade as was damaged too badly to save; reinstalled door. installed a small piece of stainless where the door meets the wall at bottom to hide scratched up laminate; do not stock a key lock for door. ....CONTINUED NEXT PAGE --added a strap to go around the Welch-Allyn equipment to keep it from falling out during travel --we made a flip down counter top for rear room had to install an overlay panel to wall so the countertop brackets have something to mount to TOTAL SALES TAX FREIGHT BALANCE $3,500.00 Not Included $3,500.00 Family and Human Services Committee Roll Call District District 7 - Hyde Park and Pleasant Valley Name Truitt* District 25 - Amenia, Washington, Pleasant Valley & Millbrook Houston* District 3 - LaGrange Polasek* District 16 - Fishkill and Beacon Valdés Smith* District 9 - City of Poughkeepsie Atkins* District 2 - LaGrange, Pleasant Valley & Town of Poughkeepsie Travelpiece District 5 - Town of Poughkeepsie D'Aquanni District 8 - City and Town of Poughkeepsie Brendli District 11 - Clinton, Pleasant Valley, and Rhinebeck Kearney District 17 - Town and Village of Fishkill McHoul (VC) District 19 - North East, Stanford, Pine Plains, Milan & Red Hook Drago District 22 - Beekman and Union Vale Garito (C) Present: Absent: Vacant: L Resolution: Motion: Total : Abstentions: Yes No Absent Yes No 2025129 AUTHORIZING PAYMENT OF 2024 INVOICES FROM 2025 FUNDS AS IT PERTAINS TO THE DEPARTMENT OF HEALTH SEPTEMBER 5, 2025 Roll Call Sheets District Present: Absent: Vacant: District 7 - Hyde Park and Pleasant Valley District 25 - Amenia, Washington, Pleasant Valley & Millbrook District 3 - LaGrange District 16 - Fishkill and Beacon District 9 - City of Poughkeepsie District 1 - Town of Poughkeepsie District 2 - LaGrange, Pleasant Valley & Town of Poughkeepsie District 4 - Hyde Park and Town of Poughkeepsie District 5 - Town of Poughkeepsie District 6 - Town of Poughkeepsie District 8 - City and Town of Poughkeepsie District 10 - City of Poughkeepsie District 11 - Clinton, Pleasant Valley, and Rhinebeck District 12 - East Fishkill District 13 - East Fishkill and Wappinger ' District 14 - Wappinger and Town of Poughkeepsie District 15 - Wappinger District 17 - Town and Village of Fishkill District 18 - City of Beacon and Fishkill District 19 - North East, Stanford, Pine Plains, Milan & Red Hook District 20 - Red Hook, Tivoli, and Rhinebeck District 21 - East Fishkill District 22 - Beekman and Union Vale District 23 - Pawling, Beekman and East Fishkill 23 2 Q District 24 - Dover and Pawling Resolution: _ Motion: Last Name YES Truitt Houston Polasek Valdés Smith Atkins Gorman Travelpiece Lawler D'Aquanni Kaul Brendli Johnson Kearney Metzger Paoloni Versaci Faust McHoul Page Drago Munn Caswell Garito Rolison House Total : 23 Yes Abstentions: 0 NO absent Absent No 2025129 AUTHORIZING PAYMENT OF 2024 INVOICES FROM 2025 FUNDS AS IT PERTAINS TO THE DEPARTMENT OF HEALTH SEPTEMBER 8, 2025
